As a board-certified psychiatrist, along with Nurse Practitioner Christine Abt, RN, APN, I provide general psychiatric services in the outpatient setting.  

By scrolling down within this (Our Practice) section, you will find my office locations and hours and maps to each location.  Also within this section, under the General Practice Info heading, are my practice policies concerning Appointments, Billing and Payment, Insurance Accepted, How to Bill Your Insurance for Out of Network Benefits, and Prescription Refills. If your insurance requires pre-authorization of psychiatric services, please note the relevant information in this section.

Under the Patient Guide tab (the tabs are at the top of the page) there are a number of forms that you can download to make your care more efficient and effective.  All of the forms that you need to fill out for your initial visit are there, so that you can fill them out at your convenience prior to coming to my office.  In addition, you will find the brief Office Visit form that needs to be filled out for each subsequent visit.

Also in this section there are several brief questionnaires that you can download and fill out, which may be helpful for understanding and assessing your wellbeing.  They are organized by diagnosis.  They can be filled out at any time – but should be brought with you to the office for scoring and interpretation.  It is important to note that screening instruments cannot be used to conclusively make a diagnosis.  They are only designed to be suggestive –  and the results must be evaluated in a fuller clinical context.

General Practice Info



To schedule and initial appointment please call (847) 291-8810.  You may refer to the ADDRESSES section above and to OFFICE HOURS and DRIVING DIRECTIONS below to determine what is most convenient for you.  If you reach our voicemail, please leave preferred appointment times and we will do our best to accommodate you.

For follow-up appointments, it is recommended that you schedule your next appointment while in the office.  If you must call later to schedule, do so early enough to choose a day and time that are convenient for you.  If you wish to have an appointment longer than twenty minutes, please specify this to the office staff.


If you are unable to keep a scheduled appointment, notice must be given on the preceding business day.  For each missed appointment, there is a $100.00 charge.  If an appointment is cancelled within 24 hours, the charge is $50.00.  These fees are not covered by insurance and will not be waived.


Generally, payment is due at the time services are rendered.  If you are using insurance for which I am a network provider, it may only be your co-payment or that it due.

If you intend to use insurance, please be sure that you provide complete information and notify us immediately of any changes.  We are not responsible for charges if incomplete information is provided or if information is not updated with our office.

In order to receive insurance reimbursement, insurance plan coverage and benefits must be verified prior to services being rendered.  Mental health benefits (behavioral) may be managed by a different company than general medical benefits, and reimbursed at a lower rate, with a different co-pay due.


If your insurance is not one for which I am listed as a provider (see INSURANCE ACCEPTED below in this section), you still may be eligible for out-of-network benefits.  We have developed a form which will enable you to receive these benefits - without any hassles or work on your part.


Some plans require mental health services to be pre-authorized or pre-certified before they will be reimbursed. This authorization or certification is the patient's responsibility and should be received before services are provided.

Prior to your first visit, please ascertain whether your coverage requires pre-authorization for mental health services.  If it does, bring specific information concerning pre-authorization requirements with you.

Any bill not paid by insurance due to the lack of pre-authorization or pre-certification is the responsibility of the patient or his/her responsible party.


If you are covered by Medicare, please be aware that there are deductibles, co-payments, and co-insurance payments that are the patient's responsibility. It is a common misconception that if a provider accepts assignment from Medicare that the patient does not have to pay anything.  This is not true.   In fact, it is against the law for healthcare providers NOT to bill Medicare patients for balances owed by them or their responsible parties.


For account balances over 30 days, there is a $10.00 rebilling fee for each month that the balance is not paid. 


If you have a question about any bill or statement, please call our billing office at (844) 744-7220.  Leave your name, the patient’s name, the account number, your daytime and evening phone numbers, and any other pertinent details.  We will get back to you as soon as possible.


 It is important for you to be familiar with the names and dosages of all medications that you are taking. If you find yourself running low on medication, it usually means that it time (or past time) to schedule and appointment.

If you do need a refill between appointments, please contact your dispensing pharmacy and initiate the request with them. You must have an appointment scheduled to receive a refill.

If a refill is necessary due to a lost prescription, missed appointment or untimely appointment follow-up, a $30-$50 fee will be charged to process the request.

Some medications by law are not allowed to be refilled via phone or fax with a pharmacy. Please keep this in mind when monitoring your medication supply.

  • We do see patients outside of switchboard hours. Evening appointments are available on Mondays with Christine Abt, A.P.N after your initial visit. 
  • Saturday hours are available with Christine Abt, A.P.N. one Saturday per month.

Note: The office is at 135 N Arlington Heights Road, Suite 105, just north of Lake Cook Road, in the Buffalo Grove Business Park. The Business Park is on the north side of Lake Cook Road and 135 is the first building past the Chase Plaza on Arlington Heights Road. 

Downtown Chicago Hours and Notes:


  • Wednesdays 9:00 am – 6:00 pm
  • Additional days and hours by appointment only

Note: Please use the “upper lobby” (up from Michigan Ave.).  However, handicapped access is available from the lower lobby via elevator.  Parking is available in several nearby public garages, including at Lake and Wabash, at Randolph and Wabash, and on Wabash between Lake and Randolph.

Insurances Accepted
  • Blue Cross / Blue Shield PPO
  • Standard Medicare (We are OUT OF NETWORK with medicare replacement PPO, HMO and POS plans.)

For all other insurance plans:
We will work with you and your insurance company to be sure that you receive the maximum reimbursement available to you. You may be entitled to Out-Of-Network benefits.  We will help ensure that you receive these benefits.  We have simplified the process so that you can be ready to file for reimbursement upon leaving the office.

Hospital Affiliation
  • I am no longer affiliated with any hospital. If hospitalization is needed, I am able to make arrangements with one of my trusted colleagues to provide care in the program or at the location that best suited for any given situation.
